The site was designed to provide councils with
easy access to insurance information and forms for the Activity
Accident Insurance Program with Mutual of Omaha.
Enrollment forms and claim forms can be completed
online by the troop leader (or other registered adult in charge),
then printed out for appropriate duplication and council service
center approval. A
check may be forwarded to council service center along with the appropriate
enrollment form so that the entire package may be mailed (by
council) directly to Mutual of Omaha after approval. Note: Premium
payments should be made payable to United of Omaha Life Insurance
Company.
You may also access the site to get information
on how to file a claim, as well as all the information contained in
the brochure. Just log on and follow the simple instructions for
locating the information or form you wish.
If you do not wish to use the Web site, you may
still call the council service center to obtain printed
information such as claim forms and brochures.
If you choose to use the Web site, the council has
established the following guidelines in order to ensure timeliness
and accuracy of the forms:
Enrollment Forms (All Plans):
- The person completing the forms on line must
be a registered adult.
- Information on the forms must be completed and
must be 100% accurate or council service center will
return them to the originator.
- If a check is forwarded with an enrollment
form, the fee must be calculated correctly or the council service
center will return it to
the originator. The form will need to be received by the council
service center at least 2 weeks in
advance of the first day of the event.
- Forward the completed form, along with a check
(made out to United of Omaha Life Insurance Company), to council service
center for verification and final
approval at least 2 weeks in advance of the event. Do not forward
directly to Mutual of Omaha – they will return it if it has
not been approved by the council service center.
- Pay special attention to the $5 minimum
purchase indicated on the forms. If the cost of your event is
under $5, you will need to send a check for $5.
Claim Forms (All Plans):
- ALL information must be completed accurately on
the claim form on the Web site. If you do not wish to type the
form, you may print a blank copy and prepare it by hand.
- Print out completed form (if typed).
- Forward to council service center for validation along
with any bills (current procedure).
- Council staff will make copies and mail all to
Mutual of Omaha (current procedure)
